 The upstream source is distributed under the license GPL-2+, however
 it contains some LGPL-3+ and GPL-3+ material. Therefore, by combination

Comment:
 The or-chain of licenses if the one found in the qttoolbardialog.{h,cpp}
 C++ sources. Second, there is no explicit license for the icons, but the
 history of the upstream GIT repository indicates that they cames along the
 qttoolbardialog.{h,cpp} C++ sources. This is old material, and the original
 source ball could not be found.
